Symptom:
Outputs show up during setup but disappear the next time the action is opened.
Reason:
The Custom Action was not saved after the Outputs were added.
Solution:
After adding your Outputs, click Save before closing the Custom Action editor. Reopen the action and confirm that the fields appear under the Outputs list.
